TERMS AND CONDITIONS
These Terms and Conditions (referred to as “Terms”) take effect on November 1, 2024, and apply to all services provided by Rehab Nexus Canada INC, hereinafter referred to as “Rehab Nexus.”

Acceptance of the Terms
By accessing or using the “Rehab Nexus” platform, you agree to be bound by these Terms and Conditions. If you do not agree to these Terms, you must discontinue your use of the “Rehab Nexus” platform immediately. Rehab Nexus Canada INC reserves the right to alter/change these terms at any time without notice.

 

Age of Use and Eligibility
You are eligible to use the Rehab Nexus platform if you meet the following criteria:

1. You complete the registration process on the Rehab Nexus website with accurate, current, and complete information about yourself.

2. You are at least 18 years old.

3. You are located in an area where our services are available.

 

Patient Profile and Account
During registration, you will need to provide a valid email address, which will serve as your username, and create a secure password. You are responsible for keeping your account credentials confidential and for all activities that occur under your account. If you choose to store any Rehab Nexus data or information on your device, you acknowledge that Rehab Nexus is not responsible for the security of that data. Rehab Nexus will not be liable for any unauthorized use of your account by third parties. You agree to notify Rehab Nexus immediately of any unauthorized use of your account or any other security breaches. Rehab Nexus has full discretion on approving your account after it has been created; approval can be withdrawn at any time without notification. If you wish to terminate your account at any time, you can contact us at info@rehabnexus.com. Rehab Nexus may retain data associated with your account for a certain period of time before deleting the data in accordance with provincial and college rules and regulations. Rehab Nexus can deny access to its services or platform at any time without any notice. By creating a Rehab Nexus account, you agree that Rehab Nexus will occasionally communicate with you electronically and/or via phone (SMS and Call) with regards to appointment reminders, promotions, advertisements, security, privacy, and any administrative matter. Clients can opt out of our marketing and advertising campaigns by emailing Rehab Nexus Canada INC and notifying us.

 

Booking an Appointment
Appointment locations are mainly within your home, but we can accommodate offices and other suitable locations. Privacy with regards to appointments is based on the patient’s selected location. Rehab Nexus allows booking of healthcare practitioners in your area.

 

Rehab Nexus Catchment
Currently, Rehab Nexus is operational in the Greater Toronto Area (GTA).

 

Fees
Each practitioner charges a standard consultation fee that depends on the type of service you receive. As charges are non-refundable, you should be aware that payments made are final. You will receive a receipt, which you can access through your profile, for reimbursement purposes if you have insurance. Coverage varies by insurance provider and plan, and Rehab Nexus does not guarantee reimbursement outcomes by your insurer.

 

Payment Processing
Rehab Nexus processes credit card payments through Stripe, a third-party service integrated within the Jane app. Your credit card will be charged the full consultation fee after your appointment with the therapist. The fee amount will be shown at the time of booking, and a detailed list of service fees is available on the Rehab Nexus website. Payment information collected will be stored on your account for processing future payments for services and products.

 

Healthcare Practitioner/Consultants
Practitioners listed on Rehab Nexus operate independently (independent practice) and are not affiliated with Rehab Nexus in any partnership or employer/employee capacity. They are not considered legal representatives of Rehab Nexus. Rehab Nexus does not control or guarantee the qualifications, expertise, or quality of services provided by these practitioners. As such, Rehab Nexus is not responsible for any medical advice, treatment, or recommendations given by a practitioner before, during, or after your appointment. Any issues related to the practitioner’s diagnosis, treatment, advice, conduct, or other actions are not the responsibility of Rehab Nexus. By using the services of practitioners through the Rehab Nexus platform, you accept all associated risks and responsibilities.

 

Practitioners
Practitioners on the Rehab Nexus platform have confirmed their good standing with their respective professional college governing bodies. By using the platform, practitioners agree to follow the policies and regulations set by their professional bodies. They also agree to purchase and maintain their own personal liability insurance coverage for the services they provide. Although Rehab Nexus will request credential documentation from practitioners, it does not guarantee the accuracy of these credentials or the practitioner’s competence in delivering treatments.

 

Disclaimer
Rehab Nexus is not a healthcare provider and does not deliver/administer any medical services. We offer a platform for scheduling appointments with practitioners who conduct consultations at the client’s home or other appropriate locations. By using our platform, you understand that no doctor-patient relationship or similar connection exists between you and Rehab Nexus Canada INC. We do not engage in treatment or diagnosis and do not endorse or recommend any specific treatments. By accessing and using the Rehab Nexus platform and booking therapy services, you agree that you do so at your own risk. The platform and services are provided “as-is,” without any warranties or guarantees. Rehab Nexus serves solely as an intermediary platform, facilitating connections between users and practitioners. Rehab Nexus is not liable for any errors or inaccuracies in the platform or the services provided by practitioners. No guarantees are made regarding the outcomes of using the platform or practitioner services. Any advice or information given by practitioners through the Rehab Nexus platform is the sole responsibility of those practitioners, not Rehab Nexus. By using our platform, you acknowledge that any reliance on their advice or information is at your own risk. You assume all responsibility for any potential risks.

 

Appointments and Cancellation Policy
Rehab Nexus does not guarantee that practitioners will be available for your appointment. Practitioners may cancel at any time before the appointment. If a practitioner is more than 20 minutes late, you can reschedule the appointment at no extra cost. You may cancel the appointment without penalty if you do so at least 24 hours in advance. If you cancel successfully, any payments will be refunded to the credit card used for the booking. Cancellations made less than 24 hours before the appointment will incur the full consultation fee.

 

Trademarks
All logos, slogans, images, and trademarks displayed on the Rehab Nexus platform are the exclusive property of Rehab Nexus Canada INC. Any use, reproduction, or imitation of these trademarks is strictly forbidden and will be prosecuted according to provincial and federal laws.

 

Liability
Rehab Nexus will not be liable for any damages or losses. You agree not to bring any claims or allegations of any kind against Rehab Nexus and its directors related to your use of the services. You acknowledge and agree that Rehab Nexus is not responsible for any damages of any kind, including but not limited to:

1. Electronic correspondence that is delayed, lost, misdirected, or damaged.

2. Your use of any information or advice provided by a Practitioner during an appointment.

3. Your access to or inability to access Rehab Nexus.

4. Property damage or personal injury resulting from your use of Rehab Nexus or services provided by Practitioners.

5. Statements, conduct, or services of Practitioners.

6. Malpractice or substandard treatment provided by a Practitioner.

7. Your conduct during any interactions.

8. Any and all losses, damages, and actions of any kind resulting from your registration with Rehab Nexus.

 

Indemnification
You agree to indemnify and hold Rehab Nexus Canada INC., as well as its directors and contractors, harmless from any losses, liabilities, claims, damages, and costs, including attorney’s fees. This indemnification applies to issues arising from your violation of these Terms, alleged violations, your access or lack of access to Rehab Nexus, your use or inability to use Rehab Nexus, any violation of third-party rights, and any claims that Rehab Nexus caused harm to you or any third party.

 

Electronic Agreement
By using Rehab Nexus and creating an account, you electronically agree to these Terms. These Terms are binding on both you and Rehab Nexus Canada INC. This agreement supersedes all prior communications, understandings, and agreements between you and Rehab Nexus.
